Please help me with learning how to this CARE PLAN.

below is the scenario. Thank you in advance

Vitals 

BP: 80/60 

Temperature: 102.4

O2 sat %: 74

HR –120

patient is 74years old.

Experiencing shortness of breath, for the last few days.

• Ambulates with a walker
• her diaper is soiled with urine
• Bilateral low extremes and edema presented to her low extremes
• Placed on bi-pap and is now SP02 levels has increased to 95%
• expresses her having difficulty breathing.
• states she is suffering with high cholesterol.
• upon checking Finger stick reading is over 500
• past medical history states she has diabetes, high blood pressure and high cholesterol
• Alert and oriented x1
• States she Ran out of insulin but states that she rarely takes them anyway
• chest Xray shows left lower lobe imagery
• placed on bypass machine
• now saturated at 95%
• will be admitted to med surg floor because of pneumonia
