This assignment requires you to write on the topic of how you have used data analytics to improve your defined population’s health.

Page 2-4 or 5 will be the body of the paper with all requirements outlined below included.

Page 6 – will be your data analytics (DA) example, i. e. graph, table, dashboard, chart…whatever “tool,” you access to provide you with the fiscal, clinical, human resource, safety or risk information you use to improve your population’s health. If you have not developed a tool, or your organization does not have any tools, then based on your readings, develop one you think would helpful, and answer the questions accordingly. Of course tell me you are developing one, or using an existing one.

Page 7 – will be your references. There are no prescribed number of resources. Include those you used to support your discussion.

Introduction – Introduce the problem you were seeking to solve by utilizing DA. Also, discuss DA and why its use is important to helping you solve your challenge.

Background – Provide information regarding your challenge. Was it fiscal, clinical, H.R., or perhaps a quality issue you were hoping to solve/address. Then describe which of the Quintuple aims you are addressing, pop health, employee or patient satisfaction, reduction of health care costs, staff satisfaction/well-being or equity. etc. etc.

Discussion – Who did you engage to help you? What intra-disciplinary team members, key stakeholders did you involve in your quest? What data/sources did you collect to run the analytics on?

Outcome – did the data analysis provide you with useful information? Any tweaks made to the data collection process? Did you use the information to make a change that positively impacted your challenge?

Summary – Tie all the elements of the paper together in a concise paragraph.
