You are an undergraduate nursing student, on a clinical placement in a respite care facility, you have been partnered with Toby, who is a Registered Nurse. Toby advises you that a new client Colleen is being admitted this morning, and you will assist him in completing her admission. Patient Profile (PP): Colleen White 71 y/o female History of Present Illness (HPI): Diagnosis of idiopathic Parkinson’s Disease (PD) 6 month ago, bilateral-hand tremors, decreased balance and multiple falls. Past Medical History: Parkinson’s Disease and Depression Medications: Recent prescription & education for Levadopa (doesn’t feel she needs it yet). Social: Non-smoker, no longer drinks alcohol Psychosocial: Colleen describes feeling lonely, isolated, and frustrated with the diagnosis, indicating signs of depression. She has avoided going to see her friends due to feeling unsteady and a fear of falling. Home: Lives with her dog, in a granny flat at the back of her daughter and son in laws house, who both work full time. Three stairs must be climbed to enter the granny flat, there are no hand rails and it is cluttered. Current Functional Status: No longer drives due to tremors, less confident with reaction time, less confident walking up or down stairs,
