An example from my own working experience is implementing “charting by exception” to avoid over-charting when things are “normal”. This definitely minimizes cognitive load because instead of charting every normal detail within the systems, we just need to chart extra when things are abnormal. This makes charting take less time, which gives us more time at the bedside to focus on taking care of our patients. It also helps lessen unnecessary details to take up more space on the charting platform. It makes it easier to see when something is abnormal with a patient because there is more detailed charting. It makes it harder to miss things in the chart that are important
