Activity 27: Skills development planning, implementation and review
Select three clients you are supporting during your placement.
For each client you will need to record evidence of being involved in the skills planning, assessment, skills development and implementation. Skills development planning should be undertaken during a meeting with the client, their family and/or carer. Implementation and review will occur after you have developed a plan with the client during the initial assessment. Use the skills development record provided to document the following information:
You will also need to answer a number of written questions for each client.
Depending on the clients you are supporting during your placement and your host organisation, you may need to select more than three clients to complete this task. For example, you may support staff with the planning and skills assessment process of three clients, then support staff implement and review the skills development plans of three different clients. Keep in mind though, that regardless of how many clients are involved, there must be clear evidence each stage identified above is completed on three different occasions with three different clients.
Note: Do not include any identifying information. Identify clients by Client 1, Client 2 etc, or use first names where these will not identify them. If you work with more than three clients, create more templates, number accordingly and complete the component of the table you were involved in – i.e. planning and skills assessment or implementation and review.
