STUDENT BEING EVALUATED PLAYS THE NURSE MANAGER ON THE DAY OF SIMULATION Setting 1: It is late at night during a particularly hectic shift. A distressed young adult female having an allergic reaction arrives in the ED. She has developed a rash and is beginning to wheeze. Doctor is the physician in charge and is new to the ED. The bedside nurse is a new nurse who is in her last week of unit orientation. Physician: Orders: Benadryl 125mg IV STAT. Bedside Nurse: prepares the medication for administration. Preceptor: has heard about the new admission and comes in—state: “How are you doing, do you need any help?” Bedside Nurse: state: “I’m just about to administer 125mg of Benadryl IV to a new admission who is having an allergic reaction”. Bedside Nurse: state: “This dose seems high do you think that’s too much? Should I ask the doctor to come back and check it? Preceptor: state: “Go ahead and follow the doctor’s order, we don’t want to bother him on this busy night.” Nearby Nurse: roll eyes and makes a face toward the preceptor but say nothing. Setting 2: Patients room The patient states, “you guys are scaring me, please call someone to help you” (characters: “act” and are required to be inappropriate) Bedside Nurse: Calls for the Charge Nurse on the intercom. When CN arrives bedside nurse states: “I don’t feel comfortable giving this amount of Benadryl IV”. Charge Nurse: reading the order for Benadryl 125mg IV from the chart- states in a loud, angry voice, posturing: “I can’t believe you bothered me for this! The doctor has ordered this dose; you are new, so you don’t get to question the doctor. You just keep your mouth shut and do your work.” Bedside Nurse: (raised, trembling voice, waving syringe around) “You shouldn’t talk to me like that, I have rights”. Preceptor: state: “Now that I think about it, I’ve never given more than 50mg IV at a time before”. Charge Nurse: “Since you two are making such a fuss, I’ll gues”For your reflection please use the following format:

Look back: At an experience or event that happened in your practice recently. Review it in your mind as . Use SIMULATION Scenario as your event.

Elaborate and describe: What happened during the situation. How did you feel and how do you think others felt? What were the outcomes? Were you surprised by what happened during the event or did it turn out as you expected?

Analyze: Identify key issues clearly. Review why the situation turned out the way it did. Why did you feel or react the way you did, and why did others feel/ react that way? If the situation or outcomes were not what you expected, consider how you could improve on them next time. This is an opportunity to really question your beliefs and assumptions and ask yourself what you value. It is also an opportunity to integrate, compare and understand the knowledge you have gained from your nursing practice in relation to nursing theory. Take this opportunity to request feedback from others.

Revise your approach based on your review of the event and decide how, or if, you will change your approach. This might involve asking others for ideas for dealing with the situation next time or how to work on a learning need. With your new learning, you may decide to try a new approach, learn more about the subject, or decide that you handled the situation very well.
