Scenario: The incident that comes to mind that was a lack of communication-related to a specific patient issue occurred one evening when I received report from the day nurse at the Detox/Rehab Substance Abuse Facility that I work at. She had advised me that a patient was to be admitted but hadn’t arrived yet. She was able to give me a brief amount of information about this patient but that was all. Once report is done and the narcotic count is reconciled, evening medications need to start being administered as many of my patients are on taper protocols that are time specific. This process usually runs until 10-10:30 pm. I start rounds at 11pm and when I arrived in a certain room, there was the new admit lying in the bed. He said a counselor brought him to the room around 5 pm so he could have dinner. Apparently, the counselor did not notify the day nurse that he was here.
