An elderly African-Canadian male presents to the emergency department (ED) with a femur fracture. EMS reported that he is bed-bound in a home that smells of feces and urine. The patient reportedly lives with his daughter, who has power of attorney (POA). She is not present at the hospital.

The patient is found to have multiple pressure ulcers on his hips, heels, and sacrum. He is nonverbal and unable to participate in the comprehensive assessment. He appears emaciated and dehydrated upon presentation. The medical provider would like to obtain informed consent for insertion of a central venous catheter for fluids, intravenous antibiotics, and monitoring. The ED nurse contacts the hospital’s risk manager to determine if the patient’s daughter has legal ability to consent. In addition, the nurse requests a social worker consult to assist with suspected abuse reporting.

The daughter is contacted via phone and denies that her father sustained a fall of any type. She claims that she turns him every 2 hours, cares for his wounds, and feeds him by hand. She states that he has a volunteer caregiver that comes into the home to assist with bathing and activities of daily living. When questioned about the fracture, she stated that her father has “brittle bone disease” and that must be the reason for the fracture.
