Scenerio:

The patient is an African American woman who is 46 years old and has presented with a chief complaint of lethargy, polyuria, and polydipsia that has been present for the past three weeks. The patient’s body mass index (BMI) was assessed to be 35 kg/m2 based on her height and weight of 5 feet 7 inches and 226 pounds, respectively. She said that she leads a sedentary lifestyle and that there is a history of diabetes on her mother’s side of the family. She also stated that there is a history of diabetes on her father’s side of the family. A random finger stick reveals that the patient’s blood glucose level is 238 mg/dL. The patient is married and has four children. In addition, the patient claimed an 18-pound weight loss over the course of the previous two months, in addition to having frequent urination and extreme thirst, particularly in the late afternoon and early evening. The patient claims that they have not been hospitalized recently and that they do not have any known food or medicine sensitivities. Her most recent menstruation occurred exactly five days ago. About three times a week, Patient enjoys going out to lunch with friends. Because of her hectic schedule at work and with her child, she does not make time for exercise. admits to consuming beer on occasion but denies ever having used tobacco or illicit drugs.

Need the following answers to be detailed as much as possible.

1. Physical exam, detail the physical exam performed that relates to the chief complaint.

2. Workup: – please include a detailed appropriate order for diagnostic work up. This includes labs, imaging, ECG, diagnostic procedures (LP), etc. Points will be deducted for overordering completely unnecessary exams.

3. Differential diagnosis: – list no fewer than four likely diagnosis. These must be relatively related to the chief complain and a way to confirm or rule out each of these must be included in the workup above.

4. Diagnosis and treatment:- include the suspected diagnosis and offer a treatment plan. This will include the following as needed: Admission decision, medications, consideration for social/economic/ religious/cultural/ethical or other such factors, procedures, rehab, etc. Must be evidence based and you should verbally cite your sources (i.e. Clinical Practice Guideline, uptodate, AHA, etc)
